FDA-approved once-weekly semaglutide (Wegovy for weight reduction and cardiovascular risk; Ozempic is the same molecule for type 2 diabetes, and its label was not read here). Wegovy label titration: 0.25 mg for 4 weeks, then 0.5, 1 and 1.7 mg at 4 weeks each, then maintenance from week 17.

Protocols

  • Weight management: recommended 2.4 mg maintenancerecommended

Label path: 0.25, 0.5, 1, 1.7 mg at 4 weeks each, then 2.4 mg, the label's recommended maintenance dose. Your prescriber may stop at 1.7 mg.

  • Weight management: 1.7 mg maintenance

Label allows 1.7 mg as the maintenance dose, chosen by response and tolerability.

Dosing

  • Weeks 1-4: 0.25 mg once weekly. Weeks 5-8: 0.5 mg. Weeks 9-12: 1 mg. Weeks 13-16: 1.7 mg (https://www.accessdata.fda.gov/drugsatfda_docs/label/2025/215256s024lbl.pdf, Table 1).
  • Maintenance from week 17: 2.4 mg (recommended) or 1.7 mg once weekly; choose by response and tolerability (label 2.3).
  • If a dose is not tolerated during escalation, consider delaying escalation for 4 weeks (label 2.2).
  • Missed dose: give as soon as possible if the next dose is more than 2 days (48 h) away; if less, skip it. After 2 or more consecutive missed doses, resume as scheduled or restart the escalation (label 2.4).

Route & timing

Route: Subcutaneous, once weekly (abdomen, thigh or upper arm); injection site and time of day can change without a dose adjustment (label 2.1).

Timing: Same day each week, any time of day, with or without meals.

Reconstitution

  • Wegovy is supplied as single-dose prefilled pens (0.25 mg/0.5 mL to 2.4 mg/0.75 mL): no reconstitution. Nothing to mix.
  • In this app 'water' is arithmetic only: a 5 mg vial placeholder in 2 mL gives 2.5 mg/mL, so 0.25 mg = 10 units and 2.4 mg = 96 units, inside a 100-unit syringe. This is not a sourced instruction.
  • No sourced reconstitution figures exist for lyophilized or compounded semaglutide: No reliable figure found in the sources; ask your prescriber or supplier.

pooled vials = ceil(0.25 x 20 / 5) = 1
discard vials = ceil(20 / floor(20)) = 1

Stability

Pen: out of the refrigerator (8-30 C / 46-86 F) up to 28 days before cap removal (label 16). Stability after reconstituting a powder vial: not found in sources.

Off-cycle

None: continuous therapy.

Why:

  • The label describes ongoing weekly maintenance; changes are made with your prescriber.

Stacking rules

  • Never draw into the same syringe as another compound; use separate syringes and distinct injection sites.
  • Do not combine with other semaglutide products or any other GLP-1 receptor agonist (label, limitations of use).
  • Hypoglycemia risk rises with insulin or insulin secretagogues; their dose may need reducing (label 5.4). Delayed gastric emptying may change absorption of oral medicines (label 7.2).

Safety

Prescription-only (FDA-approved as Wegovy and Ozempic). Compounded or 'research' semaglutide is not an FDA-approved product, and its identity, purity and sterility are not guaranteed.

Common side effects:

  • Wegovy 2.4 mg vs placebo (label Table 2): nausea 44% vs 16%, diarrhea 30% vs 16%, vomiting 24% vs 6%, constipation 24% vs 11%, abdominal pain 20% vs 10%, headache 14% vs 10%, fatigue 11% vs 5%, dyspepsia 9% vs 3%, dizziness 8% vs 4%, hair loss 3% vs 1%.
  • Resting heart rate rises by 1-4 bpm on average (label 5.9).

Stop and call your prescriber if:

  • Boxed warning: thyroid C-cell tumors in rodents. Report a neck mass, trouble swallowing, shortness of breath or persistent hoarseness. Contraindicated with personal or family history of medullary thyroid carcinoma or MEN 2.
  • Severe, persistent abdominal pain (possible pancreatitis; discontinue promptly if suspected) or suspected gallbladder disease.
  • Signs of allergic reaction (anaphylaxis, angioedema): discontinue and seek medical advice.
  • Vomiting or diarrhea with dehydration (risk of acute kidney injury).
  • New depression or suicidal thoughts (label 5.10).
  • Tell your clinicians before surgery or sedation (aspiration risk, label 5.11). Discontinue at least 2 months before a planned pregnancy (label 8.3).

Labs and monitoring:

  • Renal function if GI effects cause volume depletion.
  • Blood glucose before and during treatment in type 2 diabetes; eye monitoring if diabetic retinopathy history.
  • Heart rate at regular intervals.
